Incident Management Policy

Valencea has clear processes for identifying, responding to and reporting incidents, in line with our obligations as an NDIS support provider under the NDIS (Incident Management and Reportable Incidents) Rules.

What Is a Reportable Incident

  • The death of a participant
  • Serious injury to a participant
  • Abuse or neglect of a participant
  • Unlawful sexual or physical contact with, or assault of, a participant
  • Sexual misconduct involving a participant
  • Use of an unauthorised restrictive practice

Our Response Process

When an incident occurs, our first priority is the safety and wellbeing of everyone involved. We provide immediate support, notify next of kin or nominated contacts where appropriate, and begin an internal review to understand what happened and why.

Reporting to the NDIS Commission

Reportable incidents are notified to the NDIS Quality and Safeguards Commission within the required timeframes — within 24 hours for the most serious incidents, and within 5 business days for others — in line with NDIS requirements.

Supporting Participants After an Incident

We provide ongoing support, including access to counselling or advocacy where needed, and keep participants and families informed throughout the process.

Continuous Improvement

Every incident is reviewed to identify ways to strengthen our practices, training and safeguards, so we can reduce the likelihood of similar incidents happening again.
